Key Responsibilities of Social Workers within the NDIS
1. Assessment and Planning:
– Holistic Assessments: Conduct comprehensive assessments to understand the participant’s needs, strengths, and goals, considering their social, emotional, and environmental factors.
– NDIS Planning: Assist participants in developing their NDIS plans by identifying goals and required supports and advocating for necessary services and funding.
2. Coordination and Advocacy:
– Service Coordination: Help participants navigate the NDIS system, including coordinating between various services and supports to ensure cohesive care.
– Advocacy: Advocate for participants’ rights and needs, ensuring they receive appropriate supports and services, and addressing any barriers to accessing these services.

– Empowerment: Empower participants to take control of their lives, make informed decisions, and actively participate in their care planning and implementation.
Referral Reasons to Social Workers
– Participants experiencing stress, anxiety, depression, or other mental health issues related to their disability.
– Families or caregivers needing support to cope with the challenges of caring for a person with a disability.
2. Navigating the NDIS:
– Assistance with understanding and navigating the NDIS system, including help with applications, plan reviews, and managing funding.
– Support in advocating for necessary services and ensuring the participant’s needs are met in their NDIS plan.
3. Coordination of Services:
– Participants requiring help to coordinate multiple services and supports to ensure a seamless care experience.
– Linking participants to appropriate community resources, housing, education, employment, and other support services.
